Long term treatment with meloxicam in cats

Non-steroidal anti-inflammatory drugs (NSAIDs) are the most commonly prescribed pharmaceuticals for the treatment of pain and inflammation in humans, dogs, and horses. Meloxicam is metabolized primarily through the oxidative pathway rather than the glucuronidation pathway, an important factor in limiting toxicity in cats. The dependence of many NSAIDs on the glucuronidation pathway is part of the reason for the cat’s susceptibility to NSAID toxicity. Meloxicam is a COX-2 selective inhibitor with a COX-1/COX-2 ratio of inhibition of 2.7; thus, adverse effects of meloxicam are still of concern and primarily involve the gastrointestinal tract and kidneys due to the dependence of these organs on COX-1 for health. In the European Union, Switzerland, Australia, and New Zealand, meloxicam is licensed for long-term treatment of chronic pain in cats. In the United States, meloxicam has a black box label for avoiding its repeated use due to an increased risk of acute renal failure and death. This unacceptable risk has not been a concern in other countries, possibly due to better patient selection and the lower doses used. Regardless of licensed or extra-label use of meloxicam, previous studies have shown that it can be administered safely to aged cats with or without chronic kidney disease (CKD), at least at low therapeutic doses and provided they are clinically stable and monitored carefully.



This study builds on previous work of Gunew et al. and Gowan et al. and further examines the effect of long-term meloxicam administration on renal function in cats with and without pre-existing CKD by providing long-term survival data. A total of 82 cats were enrolled in the study of which 47 had CKD and 35 did not have evidence of overt CKD. Treatment with meloxicam did not appear to decrease longevity in cats with pre-existing CKD, as survival times (median 1608 days) were not reduced when compared with previously published data. Quality of life is a major concern for cat owners, in particular with regard to chronic painful conditions. In conclusion, these authors recommend that meloxicam should be considered as part of a therapeutic regimen in cats with chronic painful conditions, with or without overt stable CKD. [GO]

Treatment of Musculoskeletal Disease in Cats

Bennett D, Morton C: A study of owner observed behavioural and lifestyle changes in cats with musculoskeletal disease before and after analgesic therapy, J Feline Med Surg 11:997-1004, 2009.

A questionnaire-based tool was used in this study to identify behavioral and lifestyle changes that are associated with chronic pain in the cat. The changes were grouped into four behavior domains (mobility, activity, grooming and temperament). Twenty-three cats with chronic musculoskeletal pain determined by physical examination were included. Subsequently, owners of these cats were asked to complete a questionnaire before and 28 days after the start of analgesic treatment (meloxicam). A veterinary surgeon was asked to provide an independent global score before and after treatment. In this study, the baseline global scores for the veterinary assessments were very close to the owners’ scores, suggesting that owners were providing a useful clinical history that correlated well with the veterinary surgeon’s physical examination. Both owners and veterinary surgeons reported significant changes in behavior and lifestyle after analgesic therapy. The authors conclude that the changes in behavior that occurred following analgesic intervention were the consequence of treatment of pain, though they could not exclude the possibility of a placebo effect. All four domains showed a reduction in scores, while the greatest reduction in owners’ scores for each of the four domains was in the activity category. [VT]

Treatment of Feline Mammary Cancer

Mammary cancer is the third most common cancer in cats. It affects mainly female cats and the mean age at diagnosis is 10-12 years. Unfortunately, 90% of mammary tumors in cats are malignant. These cancers are locally invasive and metastasize to distant locations. Treatment options include radical surgery and various chemotherapy protocols, used either alone or with surgery. However, high quality data on the best treatment approach is lacking. This article evaluates the efficacy of a treatment combination of a COX-2 inhibitor (meloxicam), chemotherapy and surgery in 23 cats with mammary adenocarcinoma. All cats underwent surgery followed by doxorubicin-based chemotherapy. Meloxicam was given long-term, starting the day after surgery. Median survival time was 460 days and the median disease-free interval was 269 days. The survival times are similar to other studies, and do not show an advantage of combining meloxicam with chemotherapy and surgery. Prospective trials with larger numbers of cats are needed to find the optimal treatment for feline mammary cancer. [SL]

Meloxicam and Kidney Function in Cats

Goodman, L.A., et al., Effects of meloxicam on plasma iohexol clearance as a marker of glomerular filtration rate in conscious healthy cats. Am J Vet Res, 2009. 70(7): p. 826-30.

This study investigates the effect of therapeutic dosages of meloxicam on the plasma clearance of iohexol in healthy, hydrated, conscious cats fed a sodium-replete diet. Meloxicam is a popular non-steroidal anti-inflammatory drug used in cats, but there are few studies on the effect of this drug on feline kidney function. Plasma clearance of iohexol is used as a marker of renal glomerular filtration rate. Six healthy adult neutered male cats were studied in the masked, randomized, crossover study and were administered either no treatment or meloxicam. The studies were performed before the treatment began (baseline) and on the final day of treatment (5 days later). The results of the study indicated that meloxicam did not measurably alter GFR in healthy, hydrated, conscious cats. Additional studies looking at the effects of NSAIDs on GFR in cats with decreased actual or effective circulating blood volume or with renal disease is needed. [VT]

Control of Postoperative Pain in Cats

This prospective, randomized, blinded and placebo-controlled study was performed at the University of Madrid, Spain to evaluate the effect of two non-steroid anti-inflammatory drugs (tolfenamic acid and meloxicam) on control of post-operative pain in cats. Sixty-nine client owned cats undergoing ovariohysterectomy were enrolled in the study. The cats were given a dose of either tolfenamic acid (Tolfedine, Vetoquinol), meloxicam (Metacam, Boehringer Ingelheim/Merial), or placebo pre-operatively and again post-operatively. Pain and wound sensitivity were assessed using standardized scales for up to 25 hours post-operatively. The meloxicam group was less painful than controls at 6 and 22 hours post-operatively; both treatment groups were less painful than controls at 25 hours. The number of cats requiring rescue analgesia did not differ between the groups. Cats receiving tolfenamic acid or meloxicam had decreased wound sensitivity compared to controls at all time points. The researchers conclude that both tolfenamic acid and meloxicam provide a similar analgesic effect for up to 24 hours post-operatively.

Meloxicam for Feline Arthritis

Osteoarthritis is a painful condition that typically affects the elbow, hip, and shoulder joints of as many as 1 in 5 senior cats. Affected cats may have a variety of clinical signs, such as difficulty in jumping, lameness, resentment of handling and stiff gait. However, many owners do not recognize the significance of these clinical signs and may attribute them to aging changes. Although this is a common disease, reports of long-term osteoarthritis therapy in cats are rare. Non-steroidal anti-inflammatory drugs (NSAIDs) have proven efficacy in dogs and humans but there are limited published data on the use of NSAIDs in the long-term management of this condition in cats. This prospective study aimed to assess the long-term safety and palatability of oral meloxicam (Metacam, Boehringer Ingelheim Vetmedica, Inc.) and its efficacy in treating osteoarthritic pain in cats when given at a dose of 0.01 to 0.03 mg/kg once daily. Forty cats diagnosed with osteoarthritis completed the trial with a mean treatment duration of 5.8 months. Gastrointestinal upset in 2/46 (4%) cats was the only adverse effect noted. Three of the cats had stable chronic renal insufficiency (IRIS stage 3). No deleterious effect on renal function was detected in cats studied. Owners subjectively assessed treatment efficacy as good or excellent in 34/40 (85%) of cases. The results of this study showed oral meloxicam to be safe and palatable long-term treatment for osteoarthritis in cats when given with food at a dose of 0.01 to 0.03 mg/kg.
